Agreement Terms.

1.) Cancel Membership: You can cancel your own memberships in our website portal or in the Foxy and Fierce app.

App: To cancel your membership from the Foxy and Fierce App:
Click on the "Me" tab at the bottom right of the app
Click on "Products" find the membership you wish to cancel
Click "Active".
Then choose "End Membership" and "Confirm."

Website Portal: To cancel your membership from the portal :
Click on "Club Account Info" in the left side menu bar,
Click on "Products"
Click on "Membership"
Click "Active"
Then choose "End Membership and "Confirm"

2.) Client agrees there are no refunds in the event they become injured, unmotivated, unwilling to follow through on training, financial hardship, drastic lifestyle change including pregnancy, illness, serious or otherwise, decide not to follow through on your original goals or if the program was not as easy or as you specifically expected it to be.

3.) Client agrees there are NO make-up classes. If you do not utilize your class sessions they do not roll over.

4.) Client agrees that Foxy and Fierce reserves the right to edit or change the dates and times for classes.

5.) Client agrees to not come to the studio if they have symptoms of Covid 19, have
tested positive for Covid 19 and have not quarantined for a minimum of 5 days after symptoms have
subsided, or if they have been exposed to someone with Covid 19 and have not been
quarantined for 14 days.

6.) Client agrees to not come to the dojo if they have any signs of illness including
cough, fever, shortness of breath, other cold or flu like symptoms.

7.) Client agrees to take necessary sanitary precautions with equipment wiping down before and after use.


What to expect:

Reservations:
All Classes must be reserved online. Due to social distancing we require class reservations for classes.

You have the option to wear shoes in class due to safety issues while skipping rope and lifting weights. If you choose to wear shoes in class they must be sanitized upon entry.

Put your items in the cubby and have your water available. No water on the dojo floor.

Your instructor will let you know what equipment you will need for class.

A yoga mat, jump rope, ankle weights, booty bands and /or hand weights will be provided in class.

Kickboxing gloves and hand wraps are required for class.
You may purchase gloves and hand wraps at our studio. New Clients can borrow on their first day.

Although all equipment will be clean and disinfected we are taking additional precautions and having our members wipe down all equipment they will be using with a disinfectant wipe prior to class starting.


We ask that our clients exit the studio after the class.

24 Hr Notification required for cancellation to not be charged.

Liability Waiver Form
To the best of my knowledge, I am in good physical condition and fully able to participate in this
course. I am fully aware of the risks and hazards connected with the participation in
Kickboxing, Boxing, Martial Arts, Bootcamp Training, Weightlifting, or
Aerobics, including physical injury or even death, and herby elect to voluntarily participate in
said event, knowing that the associated physical activity may be hazardous to me and my
property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OR
LOSS, PROPERTY DAMAGE, OR PERSONAL INJURY, INCLUDING DEATH, that may
be sustained by me, or loss or damage to property owned by me, as a result of participation in this
course. I further certify that I am at least 18 years of age. If under 18, my parent/guardian is the
below signed.
I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE, Foxy and Fierce / Hollywood Dojo/ Seido Karate , their officers, servants, agents, and employees (hereinafter
referred to as RELEASEES) from any and all liability, claims, demands, actions and causes of
action whatsoever arising out of or related to any loss, damage, or injury, including death, that
may be sustained by me, or to any property belonging to me, while participating in physical
activity, or while on or upon the premises where the event is being conducted.
It is my expressed intent that this release and hold harmless agreement shall bind the
members of my family and spouse, if I am alive, and my heirs, assigns and personal
representative, if I am deceased, and shall be deemed as a RELEASE, WAIVE,
DISCHARGE, and CONVENTION TO SUE the above named RELEASEES. I hereby
further agree that this Waiver of Liability and Hold Harmless Agreement shall be constructed in
accordance with the laws of the State of California.
In signing this release, I acknowledge and represent that I HAVE READ THE
FOREGOING Waiver of Liability and Hold Harmless Agreement, UNDERSTAND IT
AND SIGN IT VOLUNTARILY as my own free act and deed; no oral representations,
statements or inducements, apart from the foregoing written agreements have been made; and I
EXECUTE THIS RELEASE FOR FULL, ADEQUATE AND COMPLETE
CONSIDERATION FULLY INTENDING TO BE BOUND BY SAME.
